Finding A Home
Mitch has been living in the same house for over 50 years. He wants to age in place, but his health is deteriorating and his daughter, Susan, is extremely concerned for his safety. Mitch and Susan have gone back and forth on this issue for quite a while.
After a thorough assessment, the ElderTouch Care Manager in collaboration with other medical and behavioral clinicians determined that Mitch could remain at home if certain services were coordinated to deliver appropriate care and keep him safe. Having significant knowledge of the resources that are available, the ElderTouch Care Manager effectively coordinates the required resources that are high quality, yet cost-efficient. The ElderTouch Care Manager continues to support Susan with care decisions ensuring that appropriate decisions are made.
The results are positive. Mitch is in the comfort of his home and Susan has the peace of mind that he is safe and being cared for.
