Medication Review

Ruby has been experiencing rather significant health issues. As a result, she has been seeing many different physicians who have been prescribing many medications. Her son, Owen, was concerned with the number of pills. Some of his colleagues at work had told him horror stories of adverse medication reactions.

Owen gathered the names of all of the medications and provided them to the ElderTouch Care Manager. Within 24 hours a medication management report, which includes a Pharmacist review, was provided for Ruby. The review identified an issue with two medications that could have resulted in dizziness. Thus, increasing the likelihood of a fall. With the medication management report in hand, Owen and the ElderTouch Care Manager discussed the issue with the physicians and a different drug was prescribed. Owen now submits the list of Ruby’s medication to the ElderTouch Care Manager every other month for review.

The result was significant. Ruby was saved from an adverse medication reaction and Owen has a place to turn when new medications are prescribed.
