Patient’s Rights

Patient’s Rights


+ Patient shall have the right to refuse care management services and to be informed of the implications of refusal related to benefits eligibility or health outcomes


+ Patients are educated on their rights


+ Patients are informed about the use of end of life advanced case directives when appropriate


+ Patient shall be given the opportunity to participate in their plan of care with choices regarding service options discussed


+ Patients have the right to obtain information regarding criteria for case closure and to receive notification and rationale when case management services are changed or terminated


+ Patient shall be treated without discrimination as to race, disability, gender, national origin, or creed


+ Patient shall have the right to know the names of all staff members providing care


+ Patient shall be assured of confidential treatment


+ Patient shall be treated with consideration, respect, and full recognition of his/her dignity and individuality, including privacy in treatment and care for his/her personal needs


+ Patient shall have the right to make a complaint without fear of retribution or jeopardizing his/her standard of care


+ Patient or family member will be informed of alternative approaches if they are unable to fully participate in the case management assessment phase
